How long will my first appointment be?
Your first appointment is a comprehensive new patient evaluation with dedicated time set aside to thoroughly review your medical history, understand your current health concerns, and begin building a personalized care plan. We do not rush. Your time with Dr. Shah is yours.
How do I request a referral or prior authorization?
Please contact our office directly and our team will assist you with referrals, prior authorizations, and any paperwork required by your insurance provider.
What should I bring to my first appointment?
To help us provide the best possible care, please bring the following to your first appointment:
- A valid government-issued photo ID
- Your insurance card(s)
- A list of your current medications, including dosages
- A list of your current medical conditions and diagnoses
- Any relevant medical records, laboratory results, or imaging reports (if available)
- The names and contact information of any specialists you are currently seeing
- A list of any questions or health concerns you would like to discuss
What if I need to cancel or reschedule my appointment?
We understand that life happens. Please contact us at least 24 hours in advance if you need to cancel or reschedule, so we can offer your appointment time to another patient in need.If you need to cancel or reschedule your appointment, please contact our office as soon as possible. Providing advance notice allows us to offer the appointment to another patient and helps us maintain timely access to care.
What age groups do you serve?
We provide comprehensive healthcare services for patients over the age of 12. Our practice offers personalized care for adolescents, adults, and seniors, with a focus on preventive care, chronic disease management, and long-term wellness.
What if I don't have insurance?
No Insurance, No worries!!!!
We believe that everyone deserves quality healthcare regardless of insurance coverage. Please contact our office to discuss self-pay options and payment arrangements. We are committed to working with you to ensure that cost is not a barrier to the care you need and deserve.
